Fistula Treatment Options

Fistulotomy
This is the most common and straightforward procedure, particularly suitable for simple, low-lying fistulas. The fistula tract is surgically opened and allowed to heal from the inside out. Healing typically takes a few weeks.

Seton Placement
A seton is a thin surgical thread inserted into the fistula tract to help drain infection and allow the area to heal gradually. It’s often used for complex or high-risk fistulas, or as a staging procedure before more definitive surgery.

Ideal for: Complex or multiple tracts

Purpose: Promotes drainage, reduces inflammation

Can be combined with: Later procedures such as LIFT or advancement flap
Advancement Flap Surgery
In this technique, the internal opening of the fistula is covered with a flap of healthy tissue from the rectal wall. This is a more delicate operation, often used when the fistula passes through important sphincter muscles.

Success rate: Variable, but effective in skilled hands
LIFT Procedure (“Ligation of the Intersphincteric Fistula Tract”)
An advanced, minimally invasive technique that carefully closes the fistula tract while preserving surrounding muscles. It offers an excellent balance between effectiveness and continence preservation.
